Add cost per credit and total cost to the header response

Add two new header fields to the header response:
- x-call-credits-cost: describes the cost per credit e.g. 0.15
- x-call-credits-cost-total: describes the total cost per request (x-call-credits-spent*x-call-credits-cost) e.g. 0.45
The cost of goods and services to create our product is very important to us. If we change our plan with you or you change your prices, we will need to change that in our configuration files. Having visibility into the cost programmatically is a value add for COGS tracking.
